The Meerkat method

The Meerkat method is an easy lightweight risk assessment method that is also used in the major incident draft template. The methodology can be used generically and is simple enough for multiple types of applications, even in projects. When dealing with major incidents or problems, root cause analysis is futile, unless the risk assessment is completed (or why bother?).
As an example, I once had it as a same sample survey on Survey Monkey but it is now lost to the ether.  Will recreate it one day again!

Battery Room Explosion

A hydrogen explosion occurred in an Uninterruptible Power Source (UPS) battery room. The explosion blew a 400 ft2 hole in the roof, collapsed numerous walls and ceilings throughout the building, and significantly damaged a large portion of the 50,000 ft2 building.
