Yesterday was Streve Irwin day. Our family often watch the Croc Hunter, before and after his death, and for us, Steve was and always will be a hero.

The Meerkat method is an easy lightweight risk assessment method that is also used in the major incident draft template. The methodology can be used generically and is simple enough for multiple types of applications, even in projects. When dealing with major incidents or problems, root cause analysis is futile, unless the risk assessment is completed (or why bother?). As an example, I once had it as a same sample survey on Survey Monkey but it is now lost to the ether.  Will recreate it one day again!
